What is PATHS?

PATHS is a service to address the un-met health needs of clients receiving Sickness or Invalid’s Benefit to enable them to return to work. A co-ordinator from each of the three partner agencies will work together with the client to establish the participant’s health needs and support them to access services, treatment and funding for these.

In Nelson Tasman Marlborough PATHS is a partnership between Work and Income, Kimi Hauora Wairau (The Marlborough PHO) and Te Rapuora O Te Waiharakeke. These agencies will service the entire NMDHB catchment area.

Eligibility Criteria:

  • Receiving Sickness or Invalid’s Benefit.
  • Have an UNMET physical and / or mental health need.     
  • Any motivation to work - Does not need to be linked to an actual vacancy however client must want to work.

What will a participant get?

  • A personalised health to Employment plan based on the client’s individual needs.
  • Support to access services.
  • Funding for treatment (If this treatment is an approved part of the health and to Employment plan).
  • Support to find appropriate employment.

Referral Process:

  • Discuss PATHS with your participate and ensure that they meet the above eligibility criteria.
  • Complete PATHS referral form and fax to Jo Perrett (PATHS Health Coordinator) on (03) 548 7384.
  • Jo will contact the participant and discuss the process with them.
